How do I get my hospital/clinic accredited with Generali?
We have opened the online accreditation application via our website for the convenience of interest, potential network partners. You may find the form below; just click on which type of provider (As A Hospital / Clinic / Doctor) you intend to be accredited and fill out the necessary information requested.
We offer assurance that all details submitted via the online form are secured and abides to our Privacy Policy.
An alternative to applying for accreditation is by emailing your Letter of Accreditation Intent and the required documents.

What are the requirements for accreditation with Generali?
For Hospitals:
- DOH License to Operate
- PhilHealth Accreditation Certificate

For Clinic facilities (Multispecialty/Full-service Clinic, Specialty Clinic facilities like PT Facility, Stone treatment centers, Out-patient surgical centers like Eye Centers):
- DOH License to Operate (if applicable)

How do I submit the required documents for accreditation to Generali?
Required documents for accreditation maybe submitted to Generali through the following:
- Upon filling out the accreditation form below, you will be asked to attach the necessary requirements in fulfilment of the application. Accepted attachment formats are PDF and JPEG.
- If you opt to send via email, you submit the requirements, alongside the Letter of Intent, to glapiproviderrelations@generali.com.ph Please use the email subject Accreditation Intent: <indicate if Hospital / Clinic / Doctor>
- Although we highly encourage application via online means, if scanned copies of documents are not available, hard / printed copies can be sent to the address below:

How long does it take to get accredited with Generali?
Accreditation duration will depend on the following factors:
- Submission of complete, required documents
- Review and approval time of the contract (e.g. Hospital / Clinic Contract, Medical Special Contract)
- Availability of the provider to attend the Generali Provider Network Orientation
Setting up the provider in our system will follow soon after the above – listed are fulfilled.
If I get accredited with Generali in one hospital (or clinic), can I get accredited in other hospitals or clinic as well?
The right to practice in a hospital and/or clinic is often dependent on the facility’s management’s policy.
If you are currently practicing (holding clinic) in other hospitals or clinics that are accredited with Generali, then you can also be accredited with us in those hospitals provided that you will indicate such in your Letter of Intent.
Therefore, for as long as you have a right-to-practice in another hospital or clinic accredited with Generali, then it is possible for us to accredit you in these other facilities as well.
How to I send Generali the bill or statement of account (SOA) for services rendered?
As we are going paperless and fully digital, submission of billings and relevant claims documents will be via email.

How long does it take Generali to pay its accredited providers?
We follow the 30 – day payment turnaround time, as it is with other industry players. However, please note, that the timeframe is highly dependent, as well, on the validity of the claim (e.g. with approval from Generali to perform the service rendered) and submission of complete, relevant documents. Otherwise, any missing supporting document or information may lead to possible delay in processing of the claim(s).
